PCSO - What are the requirements? 

 For the request for laboratory/Diagnostic Procedures:

1. Personal letter request addressed to General Manager, PCSO.

2. Original/Certified  True Copy of the updated Medical Abstract or Medical Certificate, signed by the Doctor with license and PTR number.

3.  Request for the laboratory/diagnostic procedure with full name and signature of the doctor and license number.

3. Official price quotation of the needed procedure from the hospital/Clinic with full name and signature of the hospital representative.

4. Endorsement from the hospital Social Service/ or Certificate of acceptance of PCSO guarantee letter. (if necessary)

 If you have questions just go  directly to PCSO or any hospital with PCSO help desk. I do not know their phone number. I just want to share this info to sick people needing  help . I got this list of  requirements when I passed by PCSO Shaw Blvd., Mandaluyong
